Immunizations
Michigan Law requires that all students be immunized against vaccine preventable diseases to attend school. Before a child can enroll in school, his/her parents/guardian must present a record of up-to-date immunizations. If a parent wishes to sign a waiver for immunizations they can be obtained at LMAS Health Department, E9526 Prospect Street in Wetmore.

Diphtheria, Tetanus & Pertussis | 4 DTP or DTAP. One dose must be on or after 4 years of age. | 4 doses D or T OR 3 doses Td if #1 given on or after age 7. 1 does of Tdap for ages 11 through 18 IF 5 years since last dose of tetnus/diphtheria containing vaccine. |
Polio | 4 doses if dose 3 adminstered on or after 4 years of age, only 3 doses are requried. | 3 doses are required. |
Measles, Mumps, Rubella | 2 doses are required. The first dose must be given on or after the 1st birthday. | 2 doses are required. The first dose must be given on or after the 1st birthday. |
Hepatitis B | 3 doses are required. | 3 doses are required. |
